MATP
Medical Assistance Transportation ProgramIf you are a Medical Assistance recipient, you may be eligible to get help with transportation to and from medical appointments and care.
The Medical Assistance Transportation Program (MATP) is a transportation service available to Medical Assistance consumers in Lackawanna County. MATP is funded by the Pennsylvania Department of Public Welfare.
MATP can help you get to medical care or services from a Medical Assistance Provider by providing transportation or mileage reimbursement.
You cannot use MATP:
- If you need emergency ambulance transportation
- For non-medical trips, such as for grocery shopping or for social activities
- To obtain medical care that is not covered by Medical Assistance
Please note that COLTS is not obligated to provide a courtesy call to the client when the van arrives within the window given when the appointment is scheduled.
Medical Transportation Options:
Depending on where you are going, what your needs are, and the cost involved, we could provide you with transportation in one of the following ways:
- Public fixed route bus (ticket would be sent in advance of your appointment)
- Shared Ride Van
- Lift-equipped vans
- Taxi
- Other
Mileage Reimbursement:
Due to strict reporting criteria the state has implemented a new mileage reimbursement form, is in effect as of MARCH, 2016. All aspects of the form needs to be filled out: name, address, MAID #, date of your appointment, appointment time, complete name and address of physician, phone number of physician, physician’s stamp or signature, parking (IF ANY), tolls (IF ANY), and total trip mileage. The form must be completed in FULL in order for the reimbursement to be paid.
The original form is required with client’s signature and date. No faxed copies will be accepted. All forms will also need a verification for each appointment the client attended. Please be advised that any future mileage forms submitted without first calling this office of said appointment no less than 1 day prior, will NOT be eligible for reimbursement. Forms must be in the office no later than the 10th of the FOLLOWING month. Confirmations received after that date may be subject to rejection for reimbursement.
